YOUR INITIAL VISIT

Paperwork intake will be provided to you on your first visit so we can gather information about you, your insurance and your attorney.
EXAMINATION
On your first visit your injuries will be examined by a licensed Chiropractic physician. The physicians at the clinic are Dr. Nathalie Blum and Dr. John Siracusa.
TREATMENT PLAN
The physician will discuss with you your treatment plan after your examination and will allow for you to ask any questions that you may have.
The Practice
At Chiropractic Injury Solutions we are focused on providing care to the injured. We work closely with your chosen attorney and are experienced with billing auto insurance. We refer to local MRI centers to further assess your injuries and refer to orthopedic, neurology, and pain management doctors to assist in your care.

Dr. Nathalie blum
Owner / Chiropractic Physician
Dr. Nathalie Blum has been practicing in Jacksonville, FL since 2008. She was born and raised in NYC, her parents are both Haitian descendants. She attended New York Chiropractic College and then later transferred to Life University in Georgia. Her career began in Florida covering Chiropractic offices which allowed her to gain experience and knowledge in owning and running her own chiropractic clinics. Currently, she owns and manages two clinics in Jacksonville FL, and one in Palm Coast FL. She is the mother of two boys named Max and Noah. Her hobbies include traveling, spending time outdoors, and enjoys eating cuisine from different cultures.
Dr. John Siracusa
CHIROPRACTIC PHYSICIAN
Dr. John Siracusa was born and raised on Long Island NY. He attended New York Chiropractic College and has been practicing in south Florida for 26 years. Dr. Siracusa also attended Palm Beach Atlantic University where he received a doctorate in pharmacology. He has a passion for helping people with various musculoskeletal disorders and has an extensive knowledge in patient care. He moved to Ponte Vedra with his family in 2020. He enjoys deep sea fishing, golfing and spending time with his wife and two daughters. He also enjoys participating in the Loggerhead Triathlon which benefits marine life, ocean and sea turtle conservation.

Yamelly Ortiz, originally from Puerto Rico, has been living in Jacksonville since 2010. She has been a Certified Medical Assistant for 9 years. Yamelly loves road trips and spending time with her 2 beautiful daughters.
